What it is

L-carnitine is an endogenous amino-acid derivative essential for fatty-acid oxidation. Oral absorption tops out around 18%, which is why injectable forms are used when therapeutic plasma levels are the goal. Clinical use includes carnitine deficiency, dialysis-related deficiency, and adjunctive endurance/recovery work.

Mechanism

Shuttles long-chain fatty acids across the inner mitochondrial membrane for β-oxidation. Injectable form bypasses oral absorption ceiling.

Outcomes it's matched to
Safety

Generally safe. High doses can cause fishy body odor (TMAO production) and mild GI upset.

  • Seizure disorder (relative)
  • Hypothyroidism (monitor)
Legal context

FDA-approved (Carnitor) for carnitine deficiency. Off-label otherwise.
